Dear Service Providers,

The DVR Service Agreement, Section 4.5. Progress Reports, includes the CSS Scanning Unit, PO Box 8927, Madison, WI 53708-8927 as an option for submitting reports to DVR.

The Scanning Unit PO Box will be permanently closed effective May 31, 2026 and will no longer serve as an option for submitting reports to DVR.

The options listed below remain available to you:

  1. The DVR Service Provider Portal (preferred)
  2. The URL Method: DVR File Upload (electronically uploading one document at a time)
  3. Fax: Statewide service reports with the DVR specialized header can be faxed to 608-327-6014. Reports without a header can be faxed toll free to 1-888-693-3479.
  4. Email: DVRSCAN@dwd.wisconsin.gov.
